If you are a member of one of the the above plans, we will file all claims.
If you are not a member of a plan with which we participate, payment
is due at time of service. We will provide you with a bill suitable for
filing with your insurance plan.
IMPORTANT
WE MUST HAVE A VALID ID CARD FROM YOUR PLAN or we must be able to verify
your information on-line with those payors that offer such functionality.
We do not accept verbal confirmation of benefits from Human Resources or
managed care plans. If you do not have a card at time of service or we cannot
verify your coverage on-line, we will expect payment at time of service. We
regret any inconvenience this may cause, however the information contained on
your insurance card or via the on-line services is vital to both our record-keeping
and claim filing operations.
